HOURS: 37 hours per week (job share will be considered)
LOCATION: Cumberland (West Cumbria)
RESPONSIBLE TO: Young carers Manager
PURPOSE OF JOB: To be responsible for identifying young carers and developing appropriate support mechanisms based on individual needs. Raising awareness of the needs of young carers with statutory and voluntary agencies.

DUTIES:
 Carry out young carer assessments and reviews using appropriate guidelines and tools
 Plan and provide direct support to young carers to meet assessed need
 To develop a range of practical/leisure/educational support services, where possible using already established agencies and organisations, to ensure best provision of service for young carers
 To keep accurate records identifying and documenting the number of young carers along with their age, location and needs/views/wishes
 To provide up to date information to young carers, their families and relevant professionals
 To provide written progress reports/project monitoring and evaluation to Carer Support West Cumbria and funders when required
 To attend carer events, meetings and forums as required
 To assist the Chief Operating Officer / Young carers Project Manager with project budget record keeping
 Develop working relationships and highlight the needs of young carers within schools throughout West Cumbria as well as other statutory/voluntary agencies
 To provide information to feed into local strategy and planning systems, to ensure that the longer term needs of young carers are highlighted
 To be aware of local, regional and national networks of support for young carers and link in when appropriate
 To work closely with all other young and adult support workers within Carer Support West Cumbria
 To work in accordance with all Carer Support West Cumbria policies and procedures
 To attend any relevant training provided by Carer Support West Cumbria
 To provide support and information within the office
 To carry out any additional relevant duties as required by this post
