CONTRACT: Initial 3 year contract
HOURS: 22.5 hours per week
LOCATION: Cumberland (West Cumbria)
RESPONSIBLE TO: Adult Carer Project Manager
PURPOSE OF JOB: To develop and implement a Life After Caring Project for former carers addressing issues such as bereavement, isolation and loneliness at an early stage. Support should comprise of engaging and inclusive activities, which encourage connections with others, cultivating an increased sense of belonging.

DUTIES:
 To make contact with former carers in such a way as to enable them to express their needs and views. Contact can be through face to face meetings, on the telephone or through virtual means
 Assist former carers to develop a plan covering the skills, strategies and tools needed following the end of their caring role. This may include financial planning, bereavement support, health and wellbeing and social opportunities, for example volunteering
 Signpost former carers to a range of practical/leisure support services, where possible using already established agencies and organisations to meet identified needs
 To identify the individual needs of former Carers and target services to meet their needs or identify alternative sources of help available
 To arrange appropriate training for former carers
 To work closely with statutory, voluntary and other agencies to enable them to make appropriate provision for the practical and emotional support of former Carers
 To enter data accurately and in a timely manner onto Carer Support West Cumbria CRM system
 To attend meetings/provide written progress reports/project monitoring and evaluation to Carer Support West Cumbria
 To work closely with all other Support Workers
 To attend any relevant training provided by Carer Support West Cumbria
 To comply with all of Carer Support West Cumbria Policies and Procedures
 To carry out any additional relevant duties as required by this post
