2x Award nominations for CVS team

Cumbria CVS staff will be on tenterhooks at the end of November, waiting to see if they have won awards at a Cumbria Learning and Improvement Collaborative (CLIC) ceremony in Carlisle.

The awards celebrate the achievements of individuals and teams within the local health and social care system across north Cumbria, with a particular focus on improvement and partnership working, and will recognise the work that has been happening in collaboration with other organisations and the people who use the services.

At the celebration event, on 30 November 2022 at People First in Carlisle, we have been shortlisted for two awards:

 

North Cumbria Living With and Beyond Cancer Project Coordinator – Jane Macfarlane

The project’s work focusses on partnership working with colleagues in health and the other third sector organisations in North Cumbria to make care more integrated, personalised and local with direct links through to a wide variety of support provided by the voluntary sector.

The aim is to ensure that people with a cancer diagnosis not only have the best clinical and psychological care but also access to the rich variety of support from the voluntary sector, helping them to overcome the many challenges they may face.

On a tiny budget, Jane has led the production of some amazing services, regularly supporting 50+ people each month.

 

Third Sector Referral Coordinator team

Over the last three years the Team have worked tirelessly to promote third sector community-based services and to support a whole range of both clinical and social care professionals to better understand the range of services and how they can support independence and enhance the health and wellbeing of patients.

Working across the whole system, they have connected with health and care partners to help develop discharge pathways, share knowledge, help identify gaps and opportunities, and connect with hundreds of grassroots third sector organisations, supporting them to better understand NHS services and connect them to appropriate staff teams.

Amongst the many positive impacts achieved, the Team have helped Both ICC and acute-based staff from across the system have a better understanding of third sector services and how they can support patients at discharge and help prevent hospital admission, and supported many more patients to feel better supported, less lonely and isolated, and have greater independence.
