Carer Support Furness looking for a self-motivated and enthusiastic individual for the post of Young Adult Carer Transition Worker. This is an exciting opportunity to work within a recognised local Charity who support Unpaid Carers. The main tasks and responsibilities of the post are to work as part of team to identify and assess the needs of Young Adult Carers who are looking after a Family Member (sibling, parent, grandparent, aunt/uncle) who cannot manage at home without this support and to help them with the transition from support as child and young person to support as an adult.
