LOCATION: Cumberland (West Cumbria)
RESPONSIBLE TO: Adult Carer Project Manager
PURPOSE OF JOB: To be responsible for carrying out annual reviews for adult carers who are registered with and have received an assessment by Carer Support West Cumbria.
DUTIES:
To undertake adult carer annual reviews, via telephone, video call or face-to-face using Carer Support West Cumbria’s procedures
Through the review process, identify individual needs including changes to contingency plans, sources of support and the need for a carer’s re-assessment
To enter necessary review and monitoring data onto Carer Support West Cumbria’s CRM system and onto Adult Social Care’s IAS System; ensuring all data is accurate and up to date
To ensure that any relevant information or concerns identified during the review are passed on accurately to the Adult Carers Project Manager or relevant staff member
To work closely with the Adult Carers Project Manager to manage and develop the adult carer review process
To provide an up to date information service for carers and their families and professionals working with carers
To comply with all of Carer Support West Cumbria Policies and Procedures
To work closely with all other staff within Carer Support West Cumbria
To attend any relevant training provided by Carer Support West Cumbria
To provide support and information within the office
To carry out any additional relevant duties as required by this post
