Salary Details: £12 per hour

One to cover North & West Cumbria

One to cover South & East Cumbria

 Main roles

To enhance the emotional wellbeing of people living with dementia so that they can continue to live as positive a life as possible.

The Community Dementia Activities Planner will:

• Receive a referral and visit the person with dementia and their family carer to help them understand the importance of continuing activity in maintaining a good level of emotional wellbeing
• Create a personalised Activity Plan for the individual
• Provide follow up contact to ensure the plan is suitable
• Set up and oversee dementia embracing community activities within their area – Community Come Dancing; Community Singing groups; Relax and Reminisce Reading groups.
